In order to give you the best possible service and so you know what to expect, 

please read through this agreement carefully, so that there are no surprises at any point.

By reading this, and continuing the process to get your appointment scheduled, you are agreeing to our NON REFUNDABLE deposit policy.    ___________________________________________________________________

What to expect-Tattoos

After continuing with this current process, please call the shop back so that we can verify your deposit and get you in the books!! On the day of your tattoo appointment, your artist will show you the design created specifically for you, (unless there has been other specifics previously discussed with your artist).  Our artists typically DO NOT draw way in advance, and will not send photos of design.  While your artist is setting up for you or drawing, please have your ID ready, sign in,  and fill out our consent form at the front desk.  Afterwards you can make any adjustments with the artwork, if need be, sit down, and get tattooed!

Afterwards you will receive aftercare and aftercare instructions, and pay for your procedure.  Feel free to check with our artists for pricing, as some of our artists have different rates and its always better to be on the same page! Your deposit will go towards the price of your tattoo.  If your tattoo will be multiple sessions, your deposit will be rolled to your next appointments, and will be deducted from your FINAL session.

 

What to expect-Piercings

After continuing with this current process, please call the shop back so that we can verify your deposit and get you in the books!!  On the day of your piercing appointment, your piercer will discuss jewelry with you and you'll be able to pick out color, size, style, etc.   

WE ARE NOT BE ABLE TO USE ANY JEWELRY BROUGHT FROM HOME OR ANYWHERE ELSE.

While our piercer is setting up for you, please have your ID ready, sign in, and fill out our consent form at the front desk.  

***If the piercing is for a Minor, PLEASE HAVE A PARENT/GUARDIAN ID AS WELL AS A COPY OF THE MINORS BIRTH CERTIFICATE!! THIS IS A MUST!!!***

Afterwards our piercer will come get you and bring you back and you will get a wonderful piercing!

Your deposit will go towards the price of your piercing.

Agreement

​

I UNDERSTAND BY CONTINUING THIS PROCESS AFTER THIS POINT THAT IN ORDER TO GET AN APPOINTMENT WITH AN ARTIST/PIERCER AT CHROMA COLLECTIVE, I MUST MAKE A DEPOSIT FOR $100 (tattoo appointment) or $25 (piercing appointment).

This deposit is NON REFUNDABLE and goes towards the final cost of my procedure.

I understand that if any of the below conditions are met, I FORFEIT my non refundable deposit, and will be required to make a new deposit to make a new appointment, or to KEEP previously scheduled additional appointments in the future.  If you do not make a new deposit, all future appointments currently scheduled will be released to waiting clients.

-Canceling my appointment.

-Failing to show up for my appointment.

-Rescheduling my appointment within 48 hours of my appointments date and time.

-Misrepresenting or failing to communicate the underage status of a client prior to making the appointment.

(we DO NOT tattoo under 18.)

-Being more than 15 minutes late for my appointment.
